This form is no longer used for the new Business Internet Banking service.
In order to reset your password, you must to answer your security questions. If you fail to answer these correctly, then you will be taken to a new password reset form.
This form is used by a Primary User to request another Activation Code. This code will be sent to you by e-mail (or post if the e-mail fails).
You should use this form if you have mislaid, already used, not received or suspended your Activation Code (used in Step 1 in the Set-up User / Business guide) following 3 unsuccessful attempts to 'Activate Business Internet Banking'.
When processed, a new Activation Code will be sent to the e-mail address submitted on this form. If we are unable to contact you via this e-mail address, the new Activation Code will be sent via the post.
The form should be printed, completed and faxed to us on 03456 037 920.
Alternatively the form can be delivered to us, via any HSBC branch.
Written requests to request an Activation Code will only be accepted on this form.
